My forum just died after doing a theme change

So yeah, basically was installing a captcha mod (http://custom.simplemachines.org/mods/index.php?mod=2932), after going into admin and making the index.template.php change the author tells us to, I get the following:
http://cudamine.com/santa-maria/forumsmf/index.php

I could still go back a page and take those lines off and save it, but no dice. I also downloaded the file through FTP and removed the lines myself, uploaded the file, still nothing.

Could really use some fast help with this...

your forum loads fine here no idea what your problem is. you are best in the future to post in the mods support thread for mod related questions.

Abort catastrophe, in panic I didn't realize the sticky on the top, all fixed apparently.
