Was My Site Hacked?

A spammer from vietnam was able to post in my forum under a locked topic with "Admin Lock Thread" Mod. I'm the only admin in my forum.

I have changed my forum admin password, cpanel and mysql. I have already checked in my forum admin panel in "Moderation Log" i can't find any trace of his IP there.

I have ask my host to look in my server log to see if there's any strange IP log there, (my couldn't find any strange IP in my server)
I'm in a dedicated server.


1. I don't know if "Admin Lock Thread" Mod is outdated?
2. I want to know how he managed to do that?

I'm using SMF defaut 1.1.14
