Need to add to Registration Form and Profile

Can someone please help. I have attempted this but not successful.

I have added this in Register.template.php

Is this correct and what else do I need to do to get this to work. I need it to show in there profile (editable) and in the email the administrator gets. Thanks any help would very much be appreciated
