[FREE] Execute php/html form in popup window and return text in message box

I've got a form where registered people should rate players in a soccer match, and I need to put those ratings in a bbc coded template. My form is generated by a php page where people can choice player, rate him, an comment that rating. a javascript function generates bbc code far that template. i need to call that form from a bbc button, and once the form is compiled it should insert bbc code into the message box.

in a future i could think to store ratings in the database, in order to create a statistics page.

It should run on smf 2.0 (actually I'm on a RC3, but i'm planning to upgrade soon)
