Application Form of some sort?

I have no idea where to post this but is there a Mod for a Customization Form fo like applications for clans and etc? If so is there a link to it?

I am not sure but I guess you can use this contact form as an Application Form..

Or you can modify  your registration form with advance profile field to act as Application + registration !
