I saw a couple of reports of this failure in the support topic for Custom Forms, with no apparent resolution.

Yea sure.

Yep. That code's from the Custom Forms mod, as Shambles suspected.

If your host uses sqlite, it seems there's a problem with that, that causes this.

Don't ask me quite what, though.
