Help me to add new BBcode code or any form near BBcode section

hello , Any one expert ,
I need to add new BBc Code or any form if submited then it will enter the data in post content area and person should be able to enter other details in post as well .


so any one urgently please help me .. where to edit and how this smf works ...

please guide me ..
