I like my bots and I don't want anything messing with my .htaccess files (except me) so I'm out.

I imagine that would slow the system down too if the .htaccess file gets too big. it couldn't be cached effectively with your continual updates.

No it will not slow your system down, the bots do that very nicely for you...
Continual Updates? That would assume that you get hit with a lot of bots per minute and if so your reasoning is a bit flawed as to what would be causing the slowness in your system.

Some bots will come to your site and a very slow bad bot can suck up 1 page every three seconds. Which is only 20 pages a minute but can hang out on your system for hours if they like if there is no deterrent. A reasonable fast bad bot can suck up maybe 10 pages a second or 600 pages a minute. Now is that what you want wasting your resources rather than a re-read of your .htaccess file. Multiple bots can suck more resources on a shared server leaving your real guest not much bandwidth...

that's not a problem for me as my forum is underutilised.

having the bots active keeps my system busier so I can  identify any performance issues etc.

if the load on my site goes up then I will consider mods such as yours.

I appreciate the time you took to explain in detail your concerns and suggestion. Here is an explanation for why the code is where it is.

index.php:

I cannot use the hooks for this because my code needs to precede the hooks. It is precisely the hooks that I need to monitor.
Being part of the hooks in the index.php file would eliminate all that I am doing entirely.

Themes:

Yes I do not like to modify themes and will look at your suggestion to see if it will get what I need done.

Register.template.php:

I cannot get the field that I need to be exactly where it needs to be other than an edit. The support issue for this I must fend.
If the field is not where it is expected to be by the bots, it will be ignored in it's entirety and there would not be a need for it.

This is an app that is to monitor SMF not become an integral part of it.
To become a part of SMF it would get lost in the shuffle and it would become ineffective at best.

You can do all the things I mentioned without an edit, it's just inconvenient. integrate_buffer receives the entire HTML before it is sent to the browser where literally any manipulation is possible. I know because I've done a number of ridiculous modifications there that shouldn't have to be modifications.

As for preceding the hooks your code needs to load a file, it really can't wait until SMF's settings are loaded to do that?

The file that you are referring to is the Settings_Client.php file, yes?
If so yes, it could load before but it definitely need to load AFTER the Settings.php. Will the integrate_pre_include guarantee this order?

Yes, you are correct. Why are there so many require_once statements in the index file if they could be handled the same way, am I missing something?

Because those require statements basically haven't changed since 1.0 before even the first iteration of integration hooks were added, originally designed for bridges but later expanded to make modding in general easier.
