Don't recognize your host, but looks like generic hosting package.  Doesn't look like you've got a clean install, still seeing mods and customizations.  Everything seems to be working, but I'd be most concerned that you got an incomplete upload of upgrade package, which would leave you with a mix of forum versions.

Could probably be a good idea to start over again, upload the files and make sure everything is actually overwritten.
Then also make sure all hooks are removed, following the instructions linked earlier.
