Try this system of questions, a dozen+, 3 or so active, all bots immediately killed the moment one clicks 'save' :O)
All done in a few minutes, 0% problems for real people registering.

Use imgination to make up questions. If (the very, very few) active human bots is any problem, keep registration approval on, or complement with something like the Stopforumspam addon.
