Template/form for posts in a specifc board?

I have tried searches but I think the search is confused by terminology!

I want to have a specific board that, by default, displays a simple template when the user selects New Topic -  typically the "headings" would be :    Date,  Location, Description, Result

The idea being that the user simply writes his text under each heading and then posts.

I have found what might be this at  http://custom.simplemachines.org/mods/index.php?mod=1279   but I'd like to check that's what it does before I try it - or if there's anything else.

1- mod specific questions should be asked in the thread dedicated to supporting that specific mod.

2- why don't you set up a test site and install the mod to test it out.

3- yes, that's what it does - but the form is a form (separate page), not part of the post screen....
