Member Approval

I am tying to approve new a member to the forum but have found I am missing quite a few options from the drop down box, such a as "approve and send e-mail."

Don't know whats happened as it's been working perfectly for years.

Looks like that particular account is pending deletion and not approval

I take it then that a member can asked to be deleted?

Yes, looks like it.
Deleting your own account is also a forum permission, you might want to look into that if it's not something you desire.

Looks like that particular account is pending deletion and not approval

or maybe they changed their email address and the forum forces them to activate after and they have not.
