Avatar removed when updating signature

Using 2.1.1 with no packages or addons aside from a different than default theme. Found here: https://anothercwot.com/memsoc/index.php

I have a couple of users reporting that their avatars are being deleted when they update their profile signature.

We have tried clearing cache, logging  out and back in, removing profile pic, saving and re-adding. Problem persists.

As Admin I cannot recreate this issue, but it is persistent. Nothing reported in error logs, either.

Edit: This seems to stem only from avatars chosen from gallery
