Automating links - lookup table?

I'm not sure where to post this question or even how to describe it properly.

A discussion group I participate in uses Simple Machines 2.0.18.  the topics often require numbered references that can be linked to URLs.  So for example, if I want to talk about item #2005, I could link '#2005' in the text to an URL that describes it.  It's always the same URL for the same item, but there are many items.

I wonder if it is possible to automate the process by for example creating a lookup table that would link the item with its URL and insert the proper link?  In other words, every time I type e.g. #2005 it would be converted to a link according to the lookup table entry for it.
