How to conserve the mods and the data?

I have the Custom profile fields mod installed in my forum. I'm using just 3 text fields to get some information regarding the organisation, course and specialisation of the member, as it being a students community.

Is there any way to conserve the mod and the data when I have to upgrade the SMF Version?
