Installing Banner_4_0.zip

SMF Version: SMF 1.0.7
I ahve a error when installing Banner_4_0.zip

it comes up all successful except last


Execute Modification ./Themes/default/index.template.php Failure

I am using default skin with my forum havent changed it yet.

Anyone know what this is?
