Real Name mod help

I have asked for a mod, but I figured I might as well create it. I have the stuff figured out, but I keep getting an error when I try to install the mod. Can anyone help please? I already have the mods done in my files by manually editing them, but others might want this. I have the zip file ready, send me an email or pm if you can help.
