Username request change

Description
I want a mod which allows people to request a username change.And the request is sent to a place to be approved or declined.

Permissions
Allows anyone to request it.

Feature Set
Admin options to approve and disapprove also a log which shows if there name has been changed before.

You could do something like that with the custom form MOD.
