I can write this as a mod for you, sure. The icons are likely to be very simple though, probably just arrows.

I also can't translate to other languages, unfortunately it'll just be in English (though if you can help me translate after I'll add that to the mod)

Specifically the strings I'd need translated would be 'Text right to left' and 'Text left to right'.

It depends how long the mod/theme outstanding queue is and whether they find any problems with the mod (there aren't any I know of)
