Looking for a mod allowing a form in a new topic

hello,

I'm looking for a mod or something which can help me doing this kind of things :
I would like to instead of "new topic" having a new "something" wich create a new topic including a form with predifined fields the users can choose.

Concretely it for a the recruitement section of a video game forum, we would like to add some areas when someone create a new topic like "Level" (low, mid, high), "class" or even "prefered map", the goal being that users can't do what they want and the board is cleanner.
The responses would be in the subjects/content of the post.

Do anyone know a mod allowing that kind of things ?

Isn't that exactly what the custom form mod does though? Make a set form, and make a topic out of it just like a sort of application form?

I interpreted the request as wanting to have fields added within the post form.
