12 and under "Permission Letter" - Due to COPPA/sensitive content in forum

Thank you in advance for looking at this problem.

Okay, here's the deal.
In Admin -> Registration -> Settings
I have checked "Age below which to apply registration restrictions" and set age to 13
"Action to take when a user below minimum age registers" = Require parent/guardian approval
"Postal address to which approval forms should be sent" = I have put an address

Problem = Form isn't laid out correctly.

Solution would be a description of how I can edit this form that is generated by my forum
