Simple Audio Video Embedder

I notice that Twitch videos are getting displayed 480x600 no matter at what default resolution is configured.
Example:

Is there any way someone can help me make this small adjustment to the code or database that would fix it?
When I edit the source manually it displays the video at the correct size:

I just don't know what file I need to adjust in order to make the fix permanent.

I uninstalled v5, tried to install 7.0.5 and I get

fclose(): Argument #1 ($stream) must be of type resource, bool given

I checked in SMF error log and php logs but could not find any further info, it points to zip file.
