Have you created both a database and a database user and associated (assigned) this user to that database? The database user must be assigned at least the following privileges on the database: SELECT, INSERT, UPDATE, DELETE, ALTER, and INDEX.

The host was contacted, and dinodom000 was provided  with the right Mysql info
