Well it looks like its a Sources problem since forcing a theme change does not do anything.

Do you remember the last mod you installed before this happened, if any?

OK, this may work..

I want you to try this, it appears to me that maybe your index.php file in root may be changed, can you find an "index.php~" in the forum main directory?

If so, after making a backup of your current index.php, rename the index.php~ to index.php and put it in place of the index.php that's currently their.

Try repairing any paths using repair_settings.php
How do I use FTP? / What is FTP?
What is repair_settings.php?

If that does not do anything, then...

It seems you have a few mods here, I see some potential issues, for a test we should upload a clean index.php, this may throw errors and possibly load incorrectly, but it should narrow the error down to index.php, if nothing changes, we can assume the issue is else where.

Get the installer package from the download area here on smf, extract only index.php from the main directory, the directory with SSI.php and subscriptions.php inside, after backing up your current index.php (you should already have a backup from earlier) place the new clean on in its place.

This is repairable without a complete restore, however, sometimes it can be more trouble than its worth, if you do go that route, may as well get all your files clean.
I would get some clean files from the installer, upload everything, except Settings.php and Settings.php~.

From their, 1st install the theme and any other themes you will want to use, then start installing mods, be sure to check for errors between installing mods.

Its best to wait at least a day before installing one mod after another. This way you can diagnose any errors in the log easier.
