Fatal error: while login to my forum

I am facing a critical error while I am log in to my forum,

Please help me to solve the Problem:

Most likely you have this:

template_init()

Present twice in a file. What mods have you installed/uninstalled recently?

Please attach:
/Sources/Load.php

And just to make sure also attach:

Themes/{current_theme}/index.template.php

OK please check attached both php files, with .txt addition

Actually till now I observed only above mentioned ID that have problem after login, or can't logged in...

the forum behaves normal with all other IDs until i logged in with mwasimr, after trying to logged in with the ID, the forum goes down, and show the same error with all IDs and even with normal browsing without logging into Forum
mean: we've to clear cache/cookies to brows again

I think the problem with mwasimr e.g.

Please attach files that have NOT been opened with any Microsoft editor. Also please DO NOT rename the files. Attach them just as they are.

These files are okay. Any chance that the user having problem selected a theme other than SMF Default Theme - Core? Would you check his theme from profile?

yes that was Core in his profile because I don't have any other, then Default - Core Theme.

This account seems to be working for me now. If it happens again, I suggest disabling template eval temporarily to see the error more clearly. Here is a tutorial about how you can do that: How to Enable / Disable eval()

Before reading above last reply I've Reset Theme Options for all members and the user successfully logged with without any error

now we can say that user: mwasimr's problem has been solved, but The "forum" problem is to be diagnosed for further disturbance (if you think it is important to know the exact error)

or mark it Solved
