Custom 'New Thread' posting options?

I'm almost certain a modification like this must exist, but I can't seem to find it, I'm sure I'm just looking for the wrong thing, but what I'm looking for is a drop down or radio button menu when posting in a certain forum (my gaming team's recruitment forum). I want the user to be able to select which game they are applying for and have their post title prefixed with [INSERTGAMEHERE] for better organization.

For example, if the prospective recruit was wanting to apply for our League of Legends team he/she would select "League of Legends" from the drop down when posting in Recruitment / Application forum. After he/she posted the thread would look like this in the Recruitment Forum index "[LoL] Pr0g4m3r's Application".

I'm just looking for a way to streamline this process... maybe the best way to do it is with just a special application page using SSI to make the post? That way I could parse all the info before it's sent to the database.
