Unable to load the 'Errors' template

We had an error with our website and are now getting this message after trying to use the repair settings tool.  before we were getting "unable to load default theme" or something to that effect.

when I replaced "cookie name" with the recommended SMFCookie139 instead of 157 I am now getting Unable to load Themes/default/index.template.php!

Got forum back up and running in 2.0.3, default theme paths were all messed up, had to fix them in db.  Patched up to 2.0.13 successfully.  When I try to install 2.0.14 patch, package manager tells me file or directory is not writeable.  All dirs are 755, files are 644, reset them to be sure.  If I let package manager connect to ftp server, it changes /root permissions to 777, which crashes forum.  (I've seen language installer do this on this host, but never package manager.)  There's no way to fix root permissions, I have to deal with chat support :P, have done this twice tonight.  Can't get into Admin -> Package Manager -> File Permissions, gives me session verification error.  Otherwise forum seems to be running normally in 2.0.13.  Bumped him up from php 5.5.22 -> 5.6.30. 

Odd that ten patches worked fine, but .14 won't go.  Any ideas?
