why is "index.php" added to URL?(and used a bookmark)

i have three SMF forums runnign, all completely updated, all running the same mods.
i use Firefox, when i navigate to each the https://www. is visible but grayed out on all of them.

One of the three exhibits this:

It also add "index.php" (grayed out) and the tab also lists the name as " - index" and as such if i or anyone bookmarks it, the book mark title is "- index" not the name of the site.

it seems this should be a setting somewhere, but i can seem to find it.
how do i change/fix this?

Everything in the address is as it should be, nothing weird there.
The "- Index" on the tab and bookmarks would look like you don't have a name set up for your forum.

I discovered that i had removed the name form the Forum Title box.  oversight .     
Ive added that but now the Tab Name and  bookmark say  "Forum name - Index"   
What confuses me is that the other two sites don't add the "  - Index" in either the tab name or the bookmark.   
Neither of the other two sites add the ".index.php" in the url either.

If you have added a mod to remove the index.php, you are going to have other problems. It is not recommended and is not necessary to remove it.

If you want the tab to show a title, you have to set the title of the web page. Go to the SMF configuration of the server and check if it has been set.

The title of the page goes in the HEADER section of the HTML. If there's no TITLE set, the browser shows the full URL or anything taken from the URL. SMF uses the server configuration field to set that.

i have not added a mod to any of the theree sites, only one shows "-index" after the title.
Al three are configured the same with the same mods.  Just odd that two dont show the -index and one does.

Yes i realized that was inadvertently removed, i have added/enabled the title , though now it shows:
"forum title -index"while the other two sites i have just show: "Forum Title"...this occurs in both the tab name and bookmark in either case
