Hello I am looking for a mod that can make people who wants to apply for my gaming community to fill in a form, rather than copying pasting and filling out a form on forum.

It would contain:

Age:

Nation:

What Game(s) are you applying for?:

What can I do for the community:

Why do I want to join =TMC= team:

Where have you heard about =TMC=:

Do you know ayone in =TMC=:

Have you eaver cheated or hacked, and please be honest:

And then when the person has filled out this form and clicked send, the form gets emailed to the recruitement team of our gaming community.

Is this possible to make and/or would it be hard and/or expensive?

I am a founder of clan, and I have coded an application form my self, its identical like the one you want.

All files are already made and the system works like a charm - ready for you to use.

Since I have put some effort into this however, I will ask for a small price.
