Forgot Password not working

I am having problem getting the email from my forum to reset the password
everything is correct even the webmasters email its not giving an error so i can figure it out

doesnt work and I made the proper changes

What do you mean by it 'doesn't work'? Did you get the 'Fail' indication or it just didn't do anything?

i only get the full code when the page loads it doesnt output the success or fail
unlike the php version check code <?php echo 'Current PHP version: ' . phpversion(); ?>

The code does work, it was used by other people before.
It's a proper php piece of code so if you get the source code displayed, make sure you uploaded a php file and not any other kind of file...
