The file upgrade.php is no exist in the installer "large upgrade"
How to upgrade from 2.0.19?

No it's not (missing that is). Just checked. It's there, just like it's supposed to.

EDIT: And to make sure I checked the bz2 as well, it's there.

Do you have the file locally in the gzip?  Some hosts have been known to block upload of some SMF files via ftp.  Usual solution is to upload via filemanager.
When in Emor, do as the Snamors.
