You can either edit index.template.php for the theme you're using, or try the Easy Edit Meta Data mod.

It looks like you are missing a image the mod added. If that is a custom theme you probably just need to navigate to your Themes/default/images/buttons and the image is probably called something similar to "warn.gif." Then you can make a copy of that image and upload it to Themes/<theme_name>/images/buttons.
