I was hoping I could fix this by adjusting code. It's pretty clear that some bit of code is repeated. I just need help in discovering where...since I am not a code meister

Actually.. I'm totally stumped. I can't see any duplicated code in any of the files you attached. And nor can Dzonny (Thanks to you btw

).

So, I advise you to uninstall it and see what happens.

Everything seems to be okay there, but chek your forum error log to see if there is some error related to this mod.
If error log is empty install mod again...

Nada in the error log. Tried re-installing & the same thing happened again. My guess is - it's clashing with another mod.

Which theme are you using? Are you edited theme of your custom theme instead of default?
