Easyforms alternative for SMF?

Hi and thanks for reading my thread. I'm new.

I recently opened an SMF forum for my gaming community. However, There is one mod for VB which I really need for SMF. Its called easyforms. Basically, you can create forms (like applications) which are then made into formal looking threads in a specific catagory.

Any ideas? :S
