Type of error: Undefined?

HI Guys getting an error about 10k times a day! it doesn't effect the forum at all but as I have to delete them I am concerned that there may be others amongst them that I miss..  The below 3 groups just keep reoccurring..  Thanks for any ideas?

have you installed any mods?

The error means exactly what it says, when called in that code, the variable $test has not yet been defined.
