Other methods to prevent spam?

I've got both httpBL and Stop Spammer installed and both streaming to their servers, visual verification and questions but I'm still getting an average of 20+ "approvals" a day.

These cannot be BOTs surely?

Can anyone tell me a better method if there is?

For me the best solution is security questions. But, ask them questions related to your forum's thematic or something that only human could now.

Also increase the delay time between posts so if spammers are posting manually they will have to spend some good time and will hopefull give up (if they have some other work to do also)
