Answer: because I'm a tool and forgot how stupid the hooks were in 2.0 (they've been improved in 2.1 significantly, and none of this code will work properly in 2.1 as a result)

Use integrate_pre_include for the filename not integrate_pre_load - pre_load is a function to call that early in the process, while integrate_pre_include actually includes the file (and I thought pre_load did that... I should have referred to the wiki article, after all I'm the one that originally wrote it )

YEAH, THANK YOU, that was it.
Now my custom file is called as expected and I can eliminate the other minor errors (db connection to third party, etc...).  When all this is done and running as expected, I'll provide the COMPLETE solution here and mark the thread as solved.
