Logging Moderators banning / unbanning activity

I have searched without any luck, so forgive me if this is something that has already been discussed.

I am having some trouble with Mods on my site removing bans that are added by an Admin, and should not be removed.  I was wondering if there is a modification, or if anyone could help me with the additional coding.

I would like if it would automatically make a log of the staff member who issues or removes a ban.  Right now it does neither, unless you simply edit a current ban.
