Questionnaire Mod?

Looked through the modifications and cannot find anything close to being suitable.

I am looking for ability to set questions and receive feedback by way of a form and post to a private board.

looked at http://custom.simplemachines.org/mods/index.php?mod=1279 but not really the solution.

If anyone knows of anything suitable please feel free to enlighten me.

a way for customers of the restaurant to feedback critique on their dining experience and environment

no big deal, it's 40c here in Turkey so i am not exactly on form either, excuse the pun.

ok thanks. i guess most people have seen a restaurant questionnaire so have the jist

radio selection, additional comment free flow text in each question (optional) and grading each question 1-5 from very bad to excellent.

i guess this would also be a nice option for polling members on any activity and admin/forum performance so it has worth for many.
