Discription:

I've seen this on other forums already and think it could be an awesome addition to my SMF 2.0 Forums. (It's mostly used for Support)
What I want:

There is a given format and you have to answer each question, this should look like this.
The Questions are created by an authorized member and only be changed in the ACP, not in the format.

Question 1 - Textbox (Admin can choose the Textboxes size)
Question 2 - Textbox (Admin can choose the Textboxes size)
Question 3 - Textbox (Admin can choose the Textboxes size)

When the questions are answered, you click on a 'Send' button and the ticket is sent to a special location, possibly something like a private message inbox of a whole group.
The user who sent the support ticket should be able to see only his ticket and its answers, there should also be a status shown like 'Unanswered' 'Read' 'Solved'.
There should be either the ability to choose a category, for example for the support "Account Recovery Problems", "Multiple Account Registration",.. , and a set one for something like Events.
The title of the Ticket should include the Username of the person who sent it and further information which could either be set by an Administrator or by the user who sends the ticket.

For every different category there should also be its own location, this could either be seperated by different buttons, or simply like the forum categories and boards are seperated.

To avoid spamming I would appreciate if there was an option like 'Only be able to send this ticket after having a minimum of 5 posts'.

I wanted to use this feature for Applications for Moderator/ Support Team and aswell as Contacting Support for help, so I'd appreciate if this feature could be set up multiple times.


Permissions/ Feature Set
Useable by anyone but guests (for some categories like Moderator Applications there should be a minimum limit of for example 5 posts setable)
Tickets can be answered by set usergroups
Tickets can only be modified and deleted by the Forum Administrator
Users are able to lock their own ticket if it's already answered.

The Administrator can create a format anytime, set questions and choose between small/ big textboxes, also the amount of Questions should be unlimited or go up to like 20.
Also Ticket Categories should also be customizeable by the Administrator.

It's not exactly what I wanted to have, but I guess I can customize it to however I like it more. ;-)

Um, it meets everything you asked for.

Good luck customising it, you'll need it, since the author wrote it a particular way and he had his reasons for so doing.
