Notifications not working

I have been asked to look into an issue with a long standing forum where the admin has now gone and I have no experience with this SMF product. The forum is running 2.1 RC2 but messages and alerts don't seem to work. SMTP and PHP Email seems to work OK as have tested it using Personal messaging, an messages sent also shows in my messages on the forum however we have setup the notification preferences to receive emails and alerts but this is not working, I have made sure that the relevant watched topics / boards are configured. There has been rumour of it very occasionally sending a number of catch up alerts in one go every so often but the general situation is that its not working. Can you advise what I should look at to try to get this resolved?

firstly update to 2.1 rc4 and see if that doesn't help

unfortunately you took over a forum that is using "beta" version
2.1 is not finalized yet, it is still in release candidate and not meant for a live environment as there will be bugs and constantly needed to be updated as fixes are made

if after updating to the latest rc version doesn't work, feel free to reply again and with any errors in your logs
