Queer problem with attachments

On our forum we have a topic where members post (embed/insert) pictures on a daily basis.

The problem is that somedays the pictures I post/insert in this topic also get attached as a thumbnail into another post in another topic. If I delete the attached thumbnail from this post the original also gets deleted.

This is happening only with me (admin) and very randomly for me to pick a pattern.

Any ideas ?

The only Mods used are Tinyportal, Welcome Topic and a custom mod which has nothing to do with attachments. This has started only after updating to 2.1.2

There are some known issues with attachments that will be addressed in 2.1.3
