registration only by the admin

is it possible to customize SMF in that way, that the admin is the only one to
register new members?

Yep. Easy to do.

Admin -> Edit Features and Options
Check the box that says "Disable member registration"

After that you can register new members by going to Admin -> Registration Management and clicking on "Register New Member"

What about membership approval?  Is this an option?

Yes it is.  It's in the same area as the other option, or should be.
