Visual verification not showing up

The "Visual verification" image is not showing up when i clicked on view image it is

showing this path with this message

The image "hxxp:www.mywebsiteforumurl.com/forum/index.php?action=verificationcode;rand=ww74188a7ab0284c40a94cea85b95a76 [nonactive]" cannot be displayed, because

it contains errors.

We have custom header with drop down for the website & footer as well

& the template what we use is display.php

so i had copied the index.php code into display.php in the middle section of the page  ( top & bottom is used to display the header & footer ) & renamed the display.php to index.php & uploaded

everything is working fine i.e it is showing the header & footer as we need when i use the original index.php it is showing up.

but it is not showing the "Visual verification" image.

Can anyone help / guide  me out in displaying the "Visual verification" image ?

I'm not sure what you mean by the template being used is display.php. Display.php is a sourcefile and has nothing to do with the templatesystem.

Can you post a link to your forum please?
Does Captcha work while using the default theme?
Did Captcha work before installing the headers and footers mod?
