Database error

while looking at old threads, I will occasionally see "empty" post. If I hit "Quick Edit" I still don't see anything in the editor window. But when I select "Modify" I will see the missing text of the post. When I go to save I get the following error.

Type of error: Database

Database Error: Duplicate entry '194389087-160816' for key 'PRIMARY'

When I hit save again, the post appears properly.

Sounds like multiple issues that might not all actually be related.
You are apparently using a custom index for search, right?
Is this an upgraded 2.0 installation? Before the upgrade, was it using UTF-8 or not?

1. Upgrade from 2.0.15
2. Not using UTF-8 prior to upgrade (as far as I know)
3. Don't know how or why on the SearchAPI-Custom.php
