I'm having this same issue with spam as well. Tons of new registered users everyday. What is the best way to go about removing them all and preventing access? You said to set it where you can have questions before they register. Is that really the best way? The constant flood of users has been a never-ending headache. Would love to get this resolved asap. The captcha for new users registering obviously doesn't work at all! I'll definitely try the questions it can't hurt. Hopefully this works!

yes. Questions are the best way to prevent spammers from registering.

as for cleanup, that's going to take a while to do right.

You can delete the users en-mass, but that leaves the posts behind -- If the posts are all in the same section, then this is easiest, since you can then set your moderation to use "checkboxes" and select the posts, again en-mass for deletion.
or
you can delete the users one by one and select to delete all of their posts when you delete the user.
