Per-user karma format (mod posted)

Following discussion elsewhere, I decided to do a little refactoring and put together per-user control of the karma display. Aside from adding a "require_once" from the templates back to Karma.php, I think the changes are pretty much stock.

I have now done a reasonable level of testing and posted the mod. Obviously it is still Beta quality, so should be used with care.

Note that the mod will leak options: a karmaModeUser in the admin settings, and a karma_mode in the per-user settings. I do not believe this will cause a problem, but if someone wants to tell me how to clean them up I will add it in.

I have completed a tool to convert GNU unified diffs into modification XML files. If this would be of use to anyone (including SimpleMachines), I can contribute it.
